Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities.PURPOSECare Coordination is an integral part of the philosophy of care at Clifford Beers Community Care Center. The Care Coordinator will deliver a blend of bilingual, family-centered, innovative billable and non-billable services to families based on their acuity and needs. Services are delivered in person, in office and in the community to facilitate the families' engagement.
