Job summary

New York Psychotherapy & Counseling Center (NYPCC) seeks a motivated Care Coordinator for our Child and Family Health Center in Brooklyn. This full-time role supports Health Home Care clients and collaborates with a multidisciplinary care team to ensure integrated services.

Requirements include a bilingual English/Spanish skill set and a Bachelor's Degree. Experience with RMA and HARP is preferred. Salary ranges from $47,000 to $60,000 per year with full benefits.

Qualifications

• Bachelor's Degree required.
• MUST be bilingual English/Spanish.
• Experience with RMA preferred.
• Experience with HARP preferred.
• Possess knowledge of various resources and community services.
• Strong verbal and written communication skills to provide linguistically appropriate services.

Responsibilities

• Manage 85+ caseload of Health Home Care clients.
• Assist in developing a Comprehensive Care Plan.
• Address housing, benefits, medical care, transportation, education, employment and other services to enhance clientQuality of life.
• Work as a member of Care Team including Supervisor, Clinicians, verbal Psychotherapists, and Psychiatrists.
• Document and monitor Electronic Health Records; create linkages/referrals.
• Contact individuals diagnosed with mental illness, substance use disorders, and chronic conditions monthly in person and by phone.
