This position assists in providing a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. Responsibilities include implementing care gap closure, supporting evidence-based medicine, managing the disease registry, supporting patient and family self-management, and coordinating care across the continuum. The Care Coordination Assistant supports Holy Cross Health’s mission through a patient-centered approach, collaborating with providers, care teams, community services, and stakeholders to deliver safe, coordinated care within the CTO network.
The role involves assisting in care delivery, managing the disease registry, supporting self-management, and facilitating communication among care teams to improve patient outcomes and promote quality improvement within the practice.
The Care Coordination Assistant collaborates with healthcare providers, care teams, and community services to ensure quality, safe, and coordinated care for patients. Key duties include overseeing the disease registry, prioritizing outreach, assisting with appointment scheduling, developing tools for optimal patient outcomes, and facilitating care team meetings.
Supports patients and caregivers in self-management, creating personalized health behavior change plans, providing education, and advocating for patients. Acts as a liaison with patients, families, care teams, and external stakeholders, including hospitalized patients.

Director, Population Health
Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) required; High school diploma required; Bachelor's degree preferred.
Bilingual Spanish speaking, five years of healthcare experience (preferably ambulatory), computer literacy, knowledge of EHR systems, Microsoft Office, performance improvement, behavioral change methodologies, and strong interpersonal skills. Ability to work autonomously and effectively in a fast-paced environment is essential.
