• Support the CHI Corps and Community Care Vans.

• Facilitate referrals and warm handoffs between patients and providers.
• Provide culturally responsive patient education on health management.
• Document patient interactions and care coordination activities accurately.
• Participate in interdisciplinary meetings and quality improvement initiatives.
• Support program performance management by monitoring KPIs.
• Maintain patient confidentiality and comply with regulatory standards.
• Perform other duties as assigned in support of the Clinical Community Programs portfolio.

What You’ll Do

• Support the Community Health Improvement (CHI) Corps and Community Care Vans
• Assist with patient outreach, care coordination, and referral management
