Job summary

Qualifications

• Master's degree in Social Work or related field, OR equivalent combination of education/experience working in hospice.
• One year experience in a related role.
• Proof of motor vehicle liability insurance upon hire and upon request.

Responsibilities

• Coordinates initial assessment with the development of a plan of care and the provision of identified services.
• Organizes and coordinates grief-related support groups and trains volunteers.
• Develops grief education programs and materials for clients and community outreach.
• Ensures visits and interventions are documented per regulatory standards.
• Performs other duties as assigned.
