Qualifications

  • Bachelor's Degree in nursing, pastoral care, social work, RTS, education or related field required.
  • 3-4 years experience in professional nursing, pastoral care, social work, bereavement, counseling or education required.
  • 2-3 years experience required.

Responsibilities

  • Evaluates grief support standards and maintains practice standards through professional organizations.
  • Provides services applying clinical counseling principles, in person or by phone, and supports nursing staff as needed.
  • Facilitates post-discharge coordination of care for patients making difficult decisions.
  • Maintains documentation in the clinical record per policies and conducts quality improvement activities.
  • Directs Bereavement Assistant work assignments and oversees grief grants utilization.
  • Conducts trainings, presentations, and after-hours support as needed.

Job Requirements

Job Responsibilities
  • Evaluates the standards of practice for grief support of grieving parents utilizing evidence based practice. Maintains practice standards through professional organizations. Ensures the timely processing of patient paperwork to facilitate external functions and processes (funerals, autopsy, transports). Resolves issues with physicians for paper work requiring physician signature. Reports issues and concerns to manager.
  • Provides services to clients applying clinical counseling principles, which may be delivered in person or on the phone. Supports nursing staff as needed. Facilitates post discharge coordination of care for patients having difficulty making decisions.
  • Demonstrates appropriate responsiveness to calls and queries from patients, families, nursing staff, physicians, pathology, and funeral homes. Resolves issues with internal and external customers through problem solving. Discusses with management to continue process improvement.
  • Maintains proper documentation in the clinical case record in compliance with TriHealth and Perinatal Policies. Holds bimonthly patient rounds with the team. Conducts QI activities to ensure the integrity of the program and care of the patient. Provides direction to the Bereavement Assistant for work assignments on scheduled work days. Oversees all grief grants and ensures utilization of the funds.
  • Provides training which may include researching and developing PowerPoint presentations on a variety of topics; also includes conducting presentations for patients, families, and Employee Orientations. Conducts support group meeting. Assists manager with after hours calls to resolve issues and concerns. May answer calls to security/pathology as needed.
  • Maintains the formal process of utilizing specific clinical standards and protocols necessary for ongoing management cases. Follows up with patients utilizing PLIDA standards of care. Ensures all encounters and attempts to contact patient are documented in EPIC as a telephone encounter for unsuccessful calls and in notes for successful calls. Utilizes smart phrases for efficient documentation. Ensures PLIDA guidelines are followed and informs management when there are problems and concerns.
