Trilogy Behavioral Healthcare is seeking a Public Benefits Coordinator in Chicago to support Medicaid and SNAP enrollment and retention, with a hybrid schedule (3 days in office, 2 days remote). You will educate clients, liaise with agencies, and monitor benefit renewals, striving for client-centered, trauma-informed care and harm-reduction approaches.

You will collaborate with clinical teams, document outcomes, and assist with transitions between benefit programs as eligibility evolves.

Qualificações

• High school diploma or GED required.
• Minimum one year of experience with mental health and/or substance use populations.
• Excellent organization and communication skills, and ability to work within our structure.

Responsabilidades

• Conduct benefits eligibility assessments for clients.
• Assist with Medicaid and SNAP applications, renewals, redeterminations and appeals.
• Maintain caseloads and monitor renewal deadlines to support retention.
• Educate clients and staff on changes in benefits and eligibility requirements.
• Document interactions and outcomes in the EHR and Availity systems.
