Qualifications

• Direct clinical services in accordance with accepted standards of social/work clinical practice.
• Plan and conduct individual, family and/or group therapy as assigned.
• Conduct, assess and interpret completion of Comprehensive Integrated Assessment.

Responsibilities

• Review all Multidisciplinary assessments and compile a Multidisciplinary Treatment Plan under supervision.
• Perform case management and discharge planning as assigned.
• Attend multidisciplinary treatment team meetings and provide updates on patients' progress toward treatment goals.
• Deliver patient services in an ethical manner promoting respect for the patient and dignity.
• Assess and interpret age/disability/cultural needs; communicate with patients and families accordingly.
• Conduct treatment and discharge planning involving the patient's support system when appropriate.
• Perform other duties and projects as assigned.
