Job summary

Prisma Health in Greenville, SC seeks a licensed clinician to provide psychosocial evaluations, psychotherapy, case management and discharge planning within Day Treatment. Qualifications

• Master’s degree in Social Work or Counseling Psychology or related field.
• One year inpatient psychiatric hospital experience and/or clinical assessments and case management experience.

Responsibilities

• Provides psychosocial evaluations, psychotherapeutic interventions, case management and discharge planning.
• Develops individualized treatment plans including discharge planning and follow up.
• Documents all social service interventions and maintains patient records in accordance with standards.
• Monitors caseload and quality review for third-party payers and regulatory agencies.
• Participates in weekend on call rotation.
