Qualifications

• Minimum 2 years of experience providing direct support to adults with disabilities or mental health conditions.
• Valid driver’s license and ability to pass DHS background study are required.
• Experience with 245D documentation and Positive Supports Rule is preferred.

Responsibilities

• Deliver person-centered services according to service support plans.
• Implement Positive Support Strategies and approved interventions.
• Assist with daily living activities and documentation.
