CROSSROADS INC OF MUSCATINE is seeking a Behavioral Support Professional to assist with daily living tasks, personal care, and safety in a residential setting for individuals with intellectual disabilities or mental health needs.

The role involves implementing IPP/ISP/BIP plans, coordinating medical appointments, transportation, and activity participation, while maintaining confidentiality and accurate records. Evening and weekend shifts are available to support client needs.

Qualifications

• Must have valid driver's license and auto insurance.

Responsibilities

• Assist in daily living tasks and personal care in a residential setting.
• Ensure personal safety and welfare of individuals with intellectual disability or mental illness.
• Implement IPP/ISP/BIP plans, HCBS guidelines.
• Take clients to medical appointments, family visits, and activities; encourage socialization.
• Maintain open communication with the Supported Community Living Manager about client progress.
• Collaborate with family, caretakers, case workers and others involved.
• Maintain records of medical visits, medications, illnesses and needs.
• Order and check in medications; administer medication, First Aid, and CPR as needed.
• Transport clients to appointments and outings around the community.
• Maintain client funds with receipts filed promptly.

Hours Available: 2nd and 3rd shift and weekends

The Behavioral Support Professional is responsible for assisting in daily living tasks, personal care, and support, and ensuring the personal safety and welfare of individuals with intellectual disability, mental illness, and/or challenging behavior in a residential setting.

• Demonstrate knowledge, understanding, and implementation of the person’s individual program plan (IPP), individual service plan (ISP), and behavior intervention plan (BIP).
• Abide by HCBS guidelines and indicated in the HCBS manual.
• Uses good decision-making skills and accesses appropriate resources to develop quality services.
• Support the needs and goals of each client by taking them to medical appointments, to visit family and friends, to preferred leisure/recreational activities, and encouraging them to socialize with their peers.
• Maintain open communication with Supported Community Living Manager to inform team members on client progress with goals, medical and/or social needs.
• Collaborate and communicate with family members, caretakers, neighbors, case workers, and other involved individuals.
• Maintain records of medical and dental visits, current medications, illnesses, injuries, and special needs.
• Order prescriptions and non-prescription medications for clients, if needed.
• Check in medications from pharmacy and administer medication, First Aid, and/or CPR if necessary.
• Transport clients to and from medical appointments and outings around the community as needed.
• Maintain client funds by ensuring receipts are recorded, and filed, in a timely fashion.
