Job summary

Family Health Centers of San Diego seeks a licensed Associate Professional Clinical Counselor to provide patient-centered care across multiple sites in San Diego County. You will assess patients, develop care plans, and deliver treatment within scope while collaborating with a multidisciplinary team.

Requirements include a Master’s degree in a related field and CA registration as an APCC, with CPR/First Aid certifications. A valid CA driver's license and insurance are also required.

Qualifications

• Master’s degree in Psychology, Counseling Psychology, Child Development or closely related field required.
• Current California registration as a Registered Associate Professional Clinical Counselor.
• CPR/First Aid certifications required; some sites exempt depending on assignment.
• Valid California driver’s license, automobile, and proof of minimum car insurance.

Responsibilities

• Complete all required documentation accurately and in a timely manner per department standards.
• Perform initial and on-going assessments for assigned patients and develop individualized care plans.
• Provide treatment within scope of practice meeting productivity and quality standards.
• Collaborate with a multi-disciplinary treatment team and communicate with providers and others.
• Document referrals and connect patients to internal/external resources.
