Assists My Sister’s Place in delivering care, observing patient behaviors, providing daily community meetings, and assisting in various procedures. Works closely with the BSW, Residential Operations Supervisor, and transdisciplinary team, reporting any changes in patient’s mood or behavior in a timely fashion. When necessary, assists the BSW and MSP staff with pre-admission screening calls from referral sources and provides information about the program to callers. Serves as a 24-hour essential employee providing direct care to residents. Supports residents in achieving their treatment plan goals, including but not limited to learning and executing daily living skills, parenting, dietary, and recovery goals. The patient population includes pregnant and parenting women and their children (0-6 years old), receiving care for substance use disorders.

Job Responsibilities:

1. Interact with co-workers, visitors, and other staff consistent with Jefferson’s values.
2. Ensure patient safety by observing and redirecting behaviors, completing scheduled rounds.
3. Utilize verbal de-escalation skills and follow protocols for managing patient behaviors.
4. Lead or assist with group activities, including community meetings, recreational groups, and meal prep. Assist residents with daily living skills and chores.
5. Monitor self-administered medications in the absence of nursing staff.
6. Communicate effectively with patients, families, staff, and other stakeholders through various channels.
7. Assist patients with activities of daily living, encouraging independence where possible.
8. Complete documentation accurately and efficiently per MSP policies and guidelines.

Minimum Education and Experience:

• High School Diploma or Equivalent required; bachelor’s degree preferred (12+ college credits in humanities or social services for Philadelphia locations).
• 1 year of related experience preferred.
• CPR certification required or must be obtained within 30 days of hire.
• Valid driver’s license with a good driving record required.

Work Shift: Rotating (United States of America)
