Job summary

Sonora Behavioral Health in Tucson, AZ is seeking a dedicated patient care assistant to support inpatient and outpatient services under supervision. You will help ensure patient safety, document clinical information, and assist with daily living activities while maintaining a caring, professional demeanor.

Responsibilities include safety checks every 15 minutes, participating in patient education and rehabilitation activities, and coordinating transportation as needed.

Qualifications

• High school diploma or equivalent required.
• Six months or more experience working with the specific population of the facility preferred.
• CPR and de-escalation and restraint certification required (training available upon hire).

Responsibilities

• Demonstrate a sense of urgency related to patient safety and provide excellent customer services.
• Ensure the well-being of patients and provide a positive, supportive and structured environment.
• Document timely, accurate and appropriate clinical information in patient's medical record.
• Assist in providing a safe, secure and comfortable environment for patients, visitors and staff.
• Interact with patients, observe behaviors and communicate significant observations to nursing staff.
• Assist patients with activities of daily living (toileting, bathing, dressing, grooming, meals).
• May obtain patient vital signs and document in patient records.
• Engage patients in educational and rehabilitative activities and group sessions.
