Job summary

PACE Southeast Michigan seeks a Behavioral Health Support Coordinator under the Behavioral Health Services department. You will coordinate services and assist with administrative duties for participants with behavioral health needs, under supervision of specialists.

Responsibilities include scheduling assessments, psychotherapy, medication management, and follow-up to ensure continuity of care. Proficiency with EHRs and MS Office is required; regular travel to SEMI centers and events is expected.

Job description

SUMMARY:

Under the leadership of the Behavioral Health Services Manager and the supervision of Behavioral Health Specialists (Master level mental health clinicians), the Behavioral Health Support Coordinator is a member of the Behavioral Health Services department responsible for coordinating services and assisting with administrative duties for PACE Southeast Michigan participants presenting with behavioral health related issues and diagnoses.

SPECIFIC DUTIES AND FUNCTIONS:

Assists with the coordination and scheduling of both internal and external behavioral health services including assessments, psychotherapy, psychiatric medication management, and inpatient treatment. Collaborates and follows up on recommendations of behavioral health specialists and/or psychiatrists to ensure adherence and favorable outcomes. Performs wellness visits/calls; monitors psychiatric symptoms and reports mental health problems or changes in status to behavioral health specialists. Assists with retrieving previous medical records to ensure continuity of care. Produces timely, complete, and accurate electronic health record documentation. Assures continuous and ongoing quality, accessibility, and safety of behavioral health services to participants. Performs other related duties, as assigned by the Manager and/or behavioral health specialists. Other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

Experience working with people with mental illness, serious mental illness, co-occurring substance use disorders, and primary health care problems. Demonstrated knowledge and skill in evaluating, planning, implementing, and monitoring patient-centered approaches to care. Knowledge of and compliance with applicable local, state, and federal laws and regulations. Cultural competence and an ability to care for a diverse population. Ability to learn and use electronic health record system. Proficient in Microsoft Word, Excel, Outlook, PowerPoint. Positive, flexible, and solution-focused attitude with ability to collaborate effectively in a team setting.

PHYSICAL REQUIREMENTS:

Prolonged periods sitting at a desk and working on a computer. Regular travel to PACE SEMI centers, community sites, and events. Ability to lift to 15 pounds as needed.
