Title: Bilingual Spanish Behavioral Health Specialist

Sign-On Bonus: $5,000

Role Description: The purpose of a Behavioral Health Specialist at Oak Street Health is to provide care management services and evidence-based short-term psychotherapy to patients with behavioral health needs. They are key resources for Care Teams across clinics, responsible for coordinating behavioral healthcare, assessing patient needs, making referrals, and providing counseling and treatments. There is potential for remote work days based on performance metrics.

Core Responsibilities:

• Collaborate with primary care teams to identify appropriate patients for behavioral health referral
• Screen and assess patients for mental health and substance use disorders
• Develop care plans and facilitate referrals to psychiatric consultants or external resources
• Provide short-term counseling and evidence-based treatments
• Coordinate care with psychiatric consultants, including telehealth handoffs
• Communicate changes in care plans with primary care teams
• Document patient encounters accurately in electronic medical records
• Participate in regional and organizational workshops and feedback sessions
• Cover multiple clinics depending on program needs
• Perform other duties as assigned

What are we looking for?

Required:

• Master's Degree in mental health counseling or related field from an accredited institution
• Highest clinical license or equivalent
• Experience with screening and assessment of mental health/substance use disorders
• Familiarity with brief, structured interventions (e.g., Motivational Interviewing, Cognitive Behavioral Therapy)
• US work authorization
• Embodies Oaky values
• Bilingual Spanish required

Strongly Preferred:

• Proficiency in brief intervention techniques
• Knowledge of differential diagnoses

• Proficiency in PC skills
