Job summary

CHI Mercy Health in Oregon is seeking a licensed Social Worker (LCSW) to advocate for clients and families, conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect patients with resources.

You will collaborate with multidisciplinary teams, support discharge planning, and apply evidence-based intervention techniques. Two years of relevant evaluation and clinical intervention experience is preferred; Master’s in Social Work is required.

Qualifications

• Must hold an Oregon LCSW license or equivalent licensure.
• Strong interpersonal and crisis intervention abilities.
• Experience in psychosocial assessments and discharge planning preferred.

Responsibilities

• Assess behavioral health needs for mental health or addiction services.
• Assess, document, and communicate risks related to self or others.
• Conduct psychosocial assessments to understand patient situations.
• Participate in and implement discharge planning.
• Provide individual, family, and group psychotherapy sessions.
• Apply evidence-based intervention techniques tailored to patients.
