Job summary

Saint Francis Hospital in Evanston, IL seeks a Social Worker to join the General Surgery department. This Full Time, Days schedule supports emotional support, crisis intervention, information and referral services for patients and families dealing with treatment, illness or trauma.

The role involves case management, milieu management, psychosocial assessments, treatment planning, and interdisciplinary discharge planning, with documentation and clinical records maintained throughout stay.

Qualifications

• Educational requirements include a Bachelor’s degree in Social Work, Marriage and Family Therapy or related field.
• A Master’s degree in psychology, social work or related field is preferred.
• Current registration as an intern or appropriate licensing requirements must be met.
• Basic Life Support (BLS) certification is required at hire and must be maintained.
• Minimal required experience includes at least 1 year in inpatient or outpatient behavioral health.

Responsibilities

• Provide emotional support, crisis intervention and counseling to patients and families.
• Provide information and referrals for financial assistance and supportive services.
• Coordinate discharge planning and complete documentation for each client.

Education

Bachelor’s degree Social Work, Marriage and Family Therapy or other related required.

Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor or related field, preferred.

Current registration an intern with Board, as appropriate to education and licensing requirements.

Current BLS certificate upon hire and maintain current.

Minimum one (1) year Inpatient or Outpatient Behavioral Health experience preferred.
