Schedule: Days (Preferred 8:00 AM – 4:30 PM; flexible within clinic hours: Mon–Thu 7 AM–7 PM, Fri 8 AM–4:30 PM)
Location: Waukesha Memorial Hospital | Remote eligible after training

ProHealth Care is seeking a Behavioral Health Screening Specialist to join our Outpatient Behavioral Medicine team. This important role serves as the first point of contact for individuals seeking behavioral health care—helping callers navigate services, schedule appointments, and conduct evidence-based screening assessments. You'll be the bridge between patients and our expert behavioral health providers, offering compassionate guidance and accurate resource coordination.

What You’ll Do

Field incoming calls from new patients seeking behavioral health services

Use screening templates and clinical knowledge to match patients with appropriate providers

Register and schedule patients for their initial appointments

Administer behavioral health screening tools (e.g., PHQ-9, GAD-7, Mini-Cog, AUDIT)

Determine the appropriate level of care, referring to Assessment & Referral team as needed

Maintain awareness of provider specialties, availability, and insurance coverage

Accurately document patient interactions in the Epic EMR

Collaborate with community providers and maintain awareness of external resources

Protect confidentiality and ensure respectful, trauma-informed interactions

What You’ll Need

Bachelor’s Degree in Psychology, Social Work, Counseling, or related Human Services field

Prior experience with behavioral health screening tools strongly preferred

Comfort with phone-based triage and patient engagement

Familiarity with Epic or other EMR systems a plus

Job Summary

The BH Screening Specialist will be responsible for managing calls from new patients wanting to establish a relationship with a BH provider and assisting callers with these decisions. They will have knowledge of the BMC providers as well as other BH resources in the community, and will make the appropriate referrals. They will register/schedule thepatientfor their first appointment with any BMC providers. They will also be trained to administer a variety of standardized BH screening tools and document accordingly in the patient's electronic medical record.

Responsibilities

Screening for outpatient appointments:

  • Fielding calls from individuals seeking an outpatient appointment with BH provider and using a templated screening tool to collect the info necessary to help them match the individual with the most appropriate BMC provider.
  • Registration/scheduling functions for registering new patients and scheduling their first appointments.
  • Knowledge of all BMC providers and of alternate providers in the community if the BMC is unable to meet the caller's needs.
  • Knowledge of the higher levels of care within the BMC and ability to determine whether a caller is in need of further evaluation for a higher level of care than the BMC Outpatient Department provides, and in these cases the ability to connect the caller to the Assessment & Referral team for further screening/assessment.
  • Knowledge of the insurances BMC providers are in network for so callers can be matched appropriately with in-network providers whenever possible.
  • Keeps current in the knowledge of community, county, state, and federal resources related to patient behavioral health care needs
  • Effectively collaborates with other departments as needed.
  • Develops/maintains positive relationships with community referral sources, i.e. employers, other treatment agencies, physicians, etc.
  • Involvement in the collection/entry/maintenance of statistical data to monitor trends in service areas as needed.

Conducting BH Screenings using Screening Tools

  • Knowledge of the BH screening tools adopted by the organization to use in screening callers/patients for depression, anxiety, subtance use disorders an other mental health conditions. These may include but are not limited to: PHQ-9, GAD-7, Mini-Cog, AUDIT.
  • Knowledge of the Screening, Brief Intervention & Referral to Treatment (SBIRT) model for screening for substance use disorders or unhealthy/at-risk substance use and ability to administer all components of the model according to protocol and in collaboration with other BMC staff members/medical providers.

Registration and Scheduling

  • Greets patients. Explains to patients what they can expect and asks them if they have any questions.
  • Inform nursing staff or provider when patients present with obvious or unusual discomfort.
  • Verifies and updates all demographic and insurance information at every visit.
  • Provides patient with all necessary forms such as but not limited to patient contact form, consent to treat, notice of privacy practices and monitors need for updates to forms per protocols.
  • Ensure that personal, confidential business and patient privacy conversations occur in a manner that is not seen or overheard by patients and visitors in the waiting room.
  • Answers phone with a standard greeting including location and name. Monitors calls on hold and wait time and checks back with the caller. Ensures all needs were met before ending the call.
  • Takes concise phones messages and routes to appropriate staff or provider.
  • Schedules patient appointments in the EMR per provider protocols and patient preferences. Offers RN triage or Assessment & Referral Triage when patient expectation cannot be met. Offer appointment reminder for patients scheduling while present in the clinic.
  • Monitor the physician referral work que daily
  • Demonstrates ability to effectively use all systems including but not limited to the EMR, Route Match, Outlook, iNet, Interpretive Services, Credit Card Banking system.
