Qualifications

• Current RN license in the state of practice.
• Willingness to obtain licenses in additional states for telehealth.
• BCLS certification; ACLS preferred for FQHC.

Responsibilities

• Perform nursing assessments and evaluate client health conditions.
• Take initial medical histories and vital signs, coordinating action plans based on results.
• Monitor health status and coordinate care with treatment teams.
• Create action plans addressing both physical and mental health treatment needs.
• Administer and monitor medications according to state and service line standards while educating clients about their benefits.
• Collaborate with physicians and consult with advanced practice providers for comprehensive client management.
• Oversee infectious disease control and ensure client hygiene practices are maintained.
• Arrange medical appointments and handle emergencies, ensuring client safety at all times.
• Provide health education and disease prevention strategies to clients.
• Complete timely documentation as required by company policies and procedures.
