Job summary

Multnomah County is hiring a Clinical Service Specialist to join the primary care treatment team. The role focuses on identifying, triaging, and managing patients with medical and behavioral health needs, providing psychoeducation, and coordinating care during behavioral health crises.

The specialist delivers brief interventions, conducts biopsychosocial assessments, and collaborates with center staff and community partners to connect patients with appropriate services.

Qualifications

  • Licensure: Licensed Clinical Social Worker (Oregon), Licensed Professional Counselor (LPC) or Psychologist (Oregon).
  • Education: Master’s degree in social work, psychology or counseling.
  • Experience: Two years as a counselor or mental health treatment professional.
  • Language: Bilingual in Spanish and English (language testing required).

Responsibilities

  • Provide brief (10‑20min), solution‑focused behavioral interventions and behavioral change planning for patients referred by medical providers.
  • Deliver follow‑up appointments to address mind/body connections related to medical presentations.
  • Conduct biopsychosocial assessments and evidence‑based psychotherapy interventions for patients across the lifespan.
  • Obtain mental health assessment, diagnosis, and brief evidence‑based psychotherapy (1–6 visits per episode) for primary care patients with mental health and addiction problems.
  • Collaborate with health center staff during psychiatric or behavioral health emergencies; lead stabilization and follow‑up when clinically appropriate.
  • Provide behavioral health consultation and education to primary care provider teams regarding psychosocial treatments.
  • Coordinate care with behavioral health community partners and maintain up‑to‑date client notes and files.
  • Identify, refer, and advocate for patients that need specialty behavioral health services.

Qualifications

Required Minimum Qualifications

  • Licensing: Licensed Clinical Social Worker (Oregon), Licensed Professional Counselor (LPC) or Psychologist (Oregon).
  • Education: Equivalent to a master’s degree in social work, psychology or counseling.
  • Professional experience: Two (2) years as a counselor, mental health treatment professional, social worker or therapist.
  • Language: Bilingual in Spanish and English (language testing will be required).
Preferred Qualifications
  • Post‑master’s level training or education in primary care behavioral health.
  • Demonstrated ability to respond effectively to crisis situations.
  • Knowledge of local behavioral health treatment system and community resources.
  • Professional demeanor and strong interpersonal communication skills.
