Job description

Job Purpose

Improve the overall health of the communities we serve by providing social or behavioral health services to patients and families.

Essential Duties and Responsibilities
  • Provide individual and group counseling services within scope of practice to assist clients in achieving more effective personal, social, educational, and vocational development and adjustment.
  • Serve as primary care behavioral health staffing and consultation resource.
  • Collect psycho‑social data about individuals and provide information to providers to assist in managing medical care.
  • Perform mental health evaluations using approved evaluation tools to assess amenability to treatment and level of care needed.
  • Develop detailed care plans for each client and document them in the electronic health record.
  • Link clients with community resources.
  • Provide in‑service training for CHAS staff as requested.
  • Participate in community outreach activities as directed.
  • For staff with chemical dependency expertise: assess clients using approved testing instruments, evaluate amenability for referral, conduct individual and group counseling and education focusing on alcohol and drug treatment, relapse prevention, and cognitive behavioral change.
  • For staff with clinical supervision training: supervise and coordinate volunteers and behavioral health/social work students as required; facilitate peer review and chart audit process as assigned.
  • Perform other duties assigned to support the CHAS Health Mission and Core Values.
Qualifications
  • Valid state licensure as a Licensed Advanced Social Worker (or associate), Licensed Independent Clinical Social Worker (or associate), Licensed Mental Health Counselor (or associate), Licensed Marriage and Family Therapist (or associate), Licensed Clinical Professional Counselor, and/or Licensed Professional Counselor.
  • Two years of clinical practice with underserved populations desired.
  • Knowledge of available community linkage resources.
  • Excellent oral and written communication skills.
  • Computer skills required.
  • Work requires independent judgment and discretion.
  • BLS (CPR/AED) required.
  • Valid driver’s license and insurance required.
  • Commitment to supporting a safe and respectful environment required.
Physical Demands
  • Talk, hear, sit more than two‑thirds of the day; use hands more than two‑thirds; reach with arms less than one‑third.
  • Communicate more than two‑thirds of the day.
  • Lifting: up to 10 pounds regularly, occasionally up to 25 pounds.
Compensation

Compensation Range: $82,387.00 - $110,722.00.
