Job Summary

Completes face‑to‑face assessments and healthcare services under the general supervision of the agency’s Medical Director. Actively participates in outstanding customer service and maintains respectful relationships with patients, families, colleagues, physicians, and referral sources.

ESSENTIAL FUNCTIONS

Evaluates health status through comprehensive health assessments as required by agency policies. Accountable for completing face‑to‑face assessments within required timeframes. Collaborates with physicians, patients/families, and interdisciplinary team regarding assessment findings, care needs, and revisions to the plan of care. Establishes and maintains positive working relationships with patients, family members, co‑workers, physicians, and referral sources.

  • Performs timely and accurate documentation in accordance with agency policy.
  • Abides by HIPAA privacy and security regulations regarding all aspects of Protected Health Information (PHI).
MINIMUM KNOWLEDGE AND SKILLS REQUIRED
  • Knowledge of MS Office software.
  • Keyboarding skills.
  • Demonstrated ability to communicate effectively.
  • Multitasking and prioritization skills.
  • Ability to apply advanced nursing principles.

LICENSURE OR REGISTRATION REQUIRED
  • Current licensure in the State of Kansas for Physician Assistant.
  • NPI Number and DEA Number.
  • Healthcare Provider BLS (AHA BLS if not American Heart Association) within three months of hire.
  • Kansas State Driver’s License.
  • Auto insurance equal to or greater than Kansas insurance requirements.
