Job summary

Pathways to Life, Inc. Behavioral Health Services – Greensboro, NC seeks a dedicated OPT Assessor to conduct comprehensive behavioral health assessments and provide clinical recommendations for appropriate levels of care.

The role partners with consumers, families, referral sources, care teams, and leadership to ensure timely access to services and regulatory compliance. The OPT Assessor will maintain strong documentation standards, HIPAA compliance, and adherence to NC Medicaid and Tailored

Qualifications

• Master’s degree preferred or equivalent experience in behavioral health assessments
• Licensure/certification as per NC Medicaid requirements (LCSW-LCSW-A, LCMHC-LCMHC-A, LCAS-LCAS-A, LMFT-LMFT-A, RN)
• Experience with NC Medicaid and Tailored Care behavioral health services is preferred
• Experience with EHR systems and strong documentation skills

Responsibilities

• Complete comprehensive behavioral health assessments and screenings
• Conduct clinical interviews and gather psychosocial history
• Determine medical necessity and recommend appropriate services
• Submit all required documentation within deadlines
• Maintain compliance with NC DHHS definitions and Medicaid requirements
• Coordinate with referral sources, care managers, hospitals, and providers
• Collaborate with internal clinical teams regarding admissions and service recommendations
• Maintain accurate electronic health record documentation
• Participate in supervision, staff meetings, and training activities
• Ensure confidentiality and HIPAA compliance at all times
• Assist with audits and quality assurance activities as needed
