Responsibilities

• Makes initial assessment visit utilizing observation/assessment skills and assists in developing the patient's plan of care in coordination with the patient's physician/allowed practitioner and Clinical Manager.
• Manages medical illness per POC; performs psycho-biological interventions.
• Conducts comprehensive medication reviews and educates patients and caregivers about medication regimens, side effects, and safety.
• Provides supportive counseling, psychoeducation, and psychotherapy as per licensure and training.
• Ensures proper documentation and compliance with regulatory standards.
• Participates in after-hours coverage, staff development, and community outreach as needed.
• Supervises care provided by LPNs and aides when indicated.
• Maintains infection control standards and proper medical waste disposal.
• Collaborates with team members to ensure quality patient care and compliance.

Qualifications

• Registered Nurse (RN) with a master's degree in psychiatric or mental health nursing and licensed in the state, with relevant experience.
• Alternatively, RN with a bachelor's degree and at least 1 year of recent psychiatric nursing experience.
• Diploma or Associate degree RN with at least 2 years of psychiatric nursing experience.
• Minimum of two (2) years of home health experience.
• Current CPR certification.
• Excellent communication, organizational, and critical thinking skills.
• Ability to work effectively in a team and independently.
