Job summary

Innovive Health seeks a Licensed Practical Nurse (LPN) to join our Home Health team in Massachusetts. You will provide high-quality nursing care to patients in their homes under the supervision of a Registered Nurse and in collaboration with an interdisciplinary care team, helping patients maintain independence and improve quality of life.

This full-time role offers a predictable Monday–Friday schedule with no overnights, weekends, or holidays.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• NCLEX-PN completed.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license and reliable transportation.
• Ability to meet health screening and clearance requirements.
• Professional references demonstrating competence.

Responsibilities

• Provide skilled nursing care per care plans.
• Administer medications and treatments safely.
• Monitor patient conditions and report to supervising RN.
• Collaborate with physicians, case managers, caregivers, and healthcare team members.
• Educate patients and families on disease management and treatment plans.
• Maintain accurate clinical documentation.
• Support patients with dignity, compassion, and respect.
• Participate in care conferences, team meetings, and ongoing training.

Required Qualifications

• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license, reliable transportation, and automobile insurance.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.

Preferred Qualifications

• Associate degree.
• Two or more years of nursing experience.
• Experience in home health, hospice, long-term care, community health, or related settings.
