Innovive Health is seeking a Licensed Practical Nurse (LPN) to join our Home Health team, providing skilled nursing care to patients in their homes under RN supervision and in collaboration with an interdisciplinary care team. You will help patients manage health, maintain independence, and improve quality of life.

This role offers a Monday–Friday schedule with a focus on meaningful one-on-one patient care, while you build lasting relationships and contribute to a supportive team environment.

Qualifikationen

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license, reliable transportation, and automobile insurance.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.
