Clarvida is seeking a Licensed Vocational Nurse or Licensed Psychiatric Technician to provide medical and psychiatric support under supervision in Anaheim, CA. You will educate clients on medications and health conditions, support care coordination for complex cases, and work with a multidisciplinary team.

The role emphasizes disease management education, adherence to HIPAA, and maintaining accurate documentation while participating in ongoing professional development and certification

Qualifications

• Graduate from an accredited vocational nursing or psychiatric technician program.
• Current LVN or LPT license in California.
• Valid California driver's license and active auto insurance.
• Minimum one year of acute care or community clinic experience preferred; perinatal care and substance use treatment experience required.

Responsabilités

• Provide case management and linkage services for clients with complex medical or psychiatric conditions.
• Follow up with physicians and pharmacies to ensure prescriptions are filled accurately and promptly.
• Administer and dispense medications under supervision of a medical provider.
• Educate clients on medications, side effects, and health conditions including TB, HIV, and hepatitis.
• Provide education on substance use and the effects of drugs and alcohol.
• Establish and maintain relationships with community resources for client support.
• Maintain accurate documentation in compliance with company and regulatory standards.
• Ensure HIPAA compliance and professional boundaries.
