Job summary

The Annex in Austin, TX seeks a Licensed Vocational Nurse (LVN) to support day-to-day clinical operations across PHP, IOP, and outpatient levels of care. You will administer medications, perform assessments, and document care in the EMR under RN supervision in a trauma-informed setting.

Ideal candidates hold a TX LVN license, BLS/CPR, and 1–3 years of behavioral health experience; residential or psychiatric settings are a plus. Evening and weekend shifts may be required.

Qualifications

• Graduate of an accredited Vocational Nursing program.
• Current, active Texas LVN license required.
• Minimum 1–3 years nursing experience; behavioral health preferred.
• BLS/CPR certification required; valid Texas driver's license.
• Experience in residential or mental health settings is desirable.

Responsibilities

• Provide direct nursing care within LVN scope in behavioral health.
• Administer medications and treatments as ordered by licensed providers.
• Monitor clients for medication effectiveness and adverse effects.
• Complete nursing assessments, vitals, and health screenings.
• Document all services and observations in the EMR.
• Support medication storage, inventory, and disposal procedures.
• Communicate concerns to RN, providers, and leadership.
• Assist with admissions, discharges, and transitions of care.
• Coordinate with therapists and case managers to support treatment plans.
• Respond to medical emergencies per program protocols.
