Job summary

Horizon Health at Reid Hospital & Health Care Services in Richmond, IN seeks an Intake Coordinator to support behavioral health admissions. You will assist with intake calls, complete paperwork, and coordinate assessments with RN, LPC or LMSW while maintaining strict confidentiality.

You will work as part of an interdisciplinary team, schedule assessments, and generate monthly reports to monitor referrals, admissions and discharges.

Qualifications

• Bachelor’s Degree in chemical dependency, psychology, social work, counseling, nursing or related field preferred.
• Master’s Degree in health or human services with practicum preferred.
• Minimum three years’ experience in a similar position.

Responsibilities

• Responds to intake calls and completes admission paperwork; assessments by RN, LPC or LMSW.
• Facilitates groups, family and individual sessions and completes documentation.
• Schedules unit assessments, meetings and appointments as appropriate.
• Uses assessment tools under supervision of RN, LPC or LMSW.
• Maintains compliance with confidentiality laws and Horizon policies.
• Generates monthly reports tracking referral, admission and discharge data.
