Job summary

The position requires a Bachelor’s in Human Services or related field; a Master’s is preferred. Certification as a Mental Health Professional (MHP) is expected; QMHP preferred. Travel to off-site locations and a valid driver’s license may be required.

Responsibilities

• Intervenes and evaluates consumers; develops treatment plans with consumer input.
• Provides crisis intervention to stabilize conditions and refers to other services as needed.
• Maintains case management and coordinates transportation and services; supports life-management skills.
