Available Locations & Shift:

Boonton/ OVERNIGHT Sunday-Tuesday: (Sunday & Monday 10pm-9am, Tuesday 11pm-9am) (32 hrs/wk)

Boonton/ Tuesday & Thursday: 11am-6pm (14 hrs/wk)

Dover/ Friday & Saturday:11pm-9am (20 hrs/wk)

Chester/ Saturday & Sunday: 11am-6pm (14 hrs/wk)

Chester/ Sunday-Tuesday: (Sunday: 9am-5pm, Monday & Tuesday: 11am-7pm) (24 hrs/wk)

Morristown/ Friday & Saturday:11am-6pm (14 hrs/wk)

Morristown/Monday-Wednesday:11am – 7pm (24 hrs/wk)

Morristown/ OVERNIGHT Thursday-Saturday: (Thursday 11pm-9am; Friday & Saturday 10pm-9am) (32 hrs/wk)

Providing direct adult rehabilitative support services to consumers in community-based transitional housing residences. (Group Home)
  • Provides on-site supervision, counseling, advocacy and assists consumers with working towards achievement of their goals as outlined in their individual rehabilitation plan.
  • Builds consumer skills and assists as needed utilizing evidenced based practice with activities of daily living, to include, but not limited to:
  • Personal hygiene;
  • Household chores, housekeeping, laundry, meal planning, shopping and meal preparation;
  • Money management/budgeting and banking;
  • Community involvement/meetings;
  • Recreational and socialization skills;
  • Access to benefits and entitlements;
  • Medical case management (assist consumers with doctor’s appointments, prescriptions, etc.);
  • Employment related activities;
  • Provide transportation in the agency vehicle in a safe, cautious and responsible manner;
  • Provides crisis intervention;
  • Provides medication supervision and tracking within agency prescribed guidelines;
  • Assists with the collection of service fees;
  • Maintains life safety documentation and standards such as fire drills, safety inspections, vehicle inspections and home related inspections at all times and reports any facility issues to maintenance for repair;
  • Participates with and assists with ensuring the environment is clean, safe and secure at all times.
Qualifications:
  • Bachelor’s degree in Mental Health or a related discipline; OR Licensed R.N.; OR two years of college plus two years of related work or life experience; OR licensed as a Practical Nurse (LPN) plus two years of related mental health experience; OR a High School diploma and four years of related mental health experience.
  • Must possess a valid driver’s license, with a good driving record.
  • Bilingual (English/Spanish) preferred, but not required.
