Qualifications

  • Experience in FQHC or community health settings preferred.
  • Experience with EHR systems preferred.

Responsibilities

Position Summary

The Behavioral Health Provider delivers comprehensive behavioral health services to patients of Health Care Partners of South Carolina. As part of an integrated healthcare team, this position provides behavioral health assessments, individual counseling, crisis intervention, treatment planning, and care coordination for patients across the lifespan. Working within a Federally Qualified Health Center (FQHC), the Behavioral Health Provider is committed to delivering compassionate, evidence-based, and patient-centered care while improving access to behavioral health services for underserved populations.

Essential Duties and Responsibilities
  • Provide behavioral health assessments, diagnostic evaluations, individual counseling, crisis intervention, and treatment planning.
  • Develop, implement, and monitor individualized treatment plans based on patient needs and evidence-based practices.
  • Coordinate care with primary care providers, behavioral health staff, nursing, pharmacy, dental, and other interdisciplinary team members.
  • Refer patients to community resources, specialty providers, inpatient treatment, and other support services as clinically appropriate.
  • Participate in integrated care initiatives that promote whole-person health and improve patient outcomes.
  • Maintain accurate, timely, and completed documentation within the Electronic Health Record (EHR).
  • Monitor patient progress, evaluate treatment effectiveness, and modify treatment plans as appropriate.
  • Maintain compliance with HIPAA, HRSA, FQHC standards, organizational policies, and applicable state and federal regulations.
  • Participate in quality improvement initiatives, provider meetings, continuing education, and departmental activities.
  • Establish and maintain collaborative relationships with community agencies and referral partners to improve access to behavioral health services.
  • Provide patient and family education regarding behavioral health conditions, treatment options, coping strategies, and community resources.
  • Perform other duties as assigned.
Education & Experience
  • Master’s degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related behavioral health field required.
  • Current South Carolina licensure in good standing as one of the following:
    • Licensed Independent Social Worker – Clinical Practice (LISW-CP)
    • Licensed Professional Counselor (LPC)
    • Licensed Marriage and Family Therapist (LMFT)
    • Licensed Addiction Counselor (LAC)
  • Minimum of one (1) year of experience providing behavioral health counseling in an outpatient, community mental health, or integrated healthcare setting preferred.
  • Experience working in a Federally Qualified Health Center (FQHC) or community health center preferred.
  • Experience using Electronic Health Record (EHR) systems preferred.
Knowledge, Skills, and Abilities
  • Knowledge of behavioral health assessment, diagnosis, counseling techniques, crisis intervention, and evidence-based treatment practices.
  • Knowledge of HIPAA regulations and the ability to maintain the confidentiality, security, and integrity of patient information.
  • Strong clinical judgment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills with the ability to establish therapeutic relationships with patients and families.
  • Ability to collaborate effectively with interdisciplinary healthcare teams and community partners.
  • Proficiency in Microsoft Office and Electronic Health Record (EHR) systems.
  • Commitment to providing compassionate, culturally competent, and patient-centered behavioral healthcare.
