Job summary

Ascension in Appleton, WI seeks a behavioral health professional to provide counseling services for child/adolescent and adult inpatient units and PHP. You will conduct assessments, set therapy goals, and deliver individual and group therapy while maintaining confidential records.

Responsibilities include evaluating progress, connecting patients with community resources, and coordinating care with families and physicians from admission to discharge. Licensure in WI and BLS preferred.

Qualifications

• Licensed clinician preferred: LPC, LCSW, or APSW.
• Wisconsin licensure/registration required prior to hire or transfer date.
• BLS Provider preferred; AHA or Red Cross accepted.
• Supervisor credentialed from the Wisconsin DSPS preferred.

Responsibilities

• Provide behavioral health counseling services to inpatient children, adolescents, and adults, including PHP.
• Conduct patient assessments and establish therapy goals.
• Administer individual and group therapy; maintain thorough, confidential records.
• Evaluate progress and effectiveness of therapy interventions.
• Provide information on community resources and activities.
• Collaborate with patient, family, physician, and community providers from treatment initiation to discharge.
