Job summary

Eventus WholeHealth seeks a behavioral health support professional to assist residents of long-term care facilities under licensed supervision. You’ll engage in assessments, education, and adherence support, with travel across regions as needed.

You will collaborate with families, staff, and other providers, documenting care and contributing to individualized treatment plans while maintaining strict confidentiality and high professional standards.

Qualifications

  • Bachelor’s degree in Counseling, Social Work, Psychology or related field required.
  • Experience in psychiatric or long-term care setting preferred.
  • Customer service experience preferred.
  • Ability to function independently and proactively in a long-term care setting.
  • Proficient with electronic medical records and Microsoft Office (Teams, Excel).
  • Strong written and verbal communication skills.
  • Ability to exercise initiative, judgment and discretion.
  • Ability to handle confidential and sensitive information.

Responsibilities

  • Assess the patient’s knowledge of their clinical condition and provide education and self-management support.
  • Provide outreach to clients who are having problems with treatment adherence.

  • Coordinate with other agencies and community providers.
  • Maintain timely documentation of work.
  • Serve as an active member of the behavioral health treatment team.

Position Summary

This individual will provide behavioral health support to residents of long term care facilities under the supervision of a licensed professional. They may receive training in a broad range of assessments and interventions that will support treatment specified in individual care plans. Regional travel will be required.

Essential Duties and Responsibilities
  • Conduct assessments that include the behavioral and social needs of the patients

Education and/or Certifications
  • Bachelor’s degree in Counseling, Social Work, Psychology or related field
Skills and Qualifications

  • Effective use of electronic medical records and Microsoft software (teams, excel spreadsheets, etc)

Physical Requirements
  • While performing the duties of this job, the employee is regularly required to communicate with others, including the ability to express oneself and exchange information in what, at times, could be a stressful or high‑pressure environment.
  • The employee frequently is required to move about the office to access files, use office equipment, and to interact with others. The person must be able to sit or remain in a stationary position for extended periods of time to be able to work, communicate on the phone, utilize the computer, and interact with others. The employee is occasionally required to stand or otherwise be able to access files, equipment, and other items in high places.
  • Employee must be able to constantly operate computers, phones, and other office equipment.
  • Employee must be able to bend, stoop, kneel, crouch, or crawl.

  • The noise level in the work environment is usually low to moderate.
  • Due to the collaborative nature of the business and the need to service customers, the employee must be able to interact effectively with others in an office environment, manage conflict, and handle stressful situations and deadlines.
