Job Description

We are seeking a Licensed Psychologist or Licensed Clinical Social Worker to work as a primary care team member, providing short-term behavioral health consultation services in a fast-paced setting. Services should uphold the health center’s mission and standards, improve patient outcomes, and support operational and fiscal capacity. The Behavioral Health Consultant reports to the Behavioral Health Network Supervisor.

Age of Patient Population Served

• Pediatric (1 - 19 yrs)
• Adult (19 – 64 yrs)
• Geriatric (65 yrs & older)

Population

• Bariatric Patients: BMI > 40 or > 35 with comorbidities
• Patients with communication needs, developmental delays, end-of-life care, isolation precautions
• All populations

Responsibilities

• Provide behavioral health consultation within primary care.
• Collaborate with primary care providers to enhance patient care, treatment planning, and monitor interventions.
• Conduct individual, couples, family, and group assessments, psychoeducation, medication adherence counseling, motivational interviewing, problem-solving, self-care planning, and substance use evaluation.
• Coordinate care, refer patients to specialists, and advocate as needed.
• Participate in crisis intervention and emergency response.
• Identify population health issues and develop strategies.
• Complete all required charting and documentation.

Job Requirements

• Licensed Clinical Social Worker or equivalent license.
• Strong engagement, assessment, and clinical skills.
• Excellent record-keeping and communication skills.
• Ability to utilize DSM-V for diagnosis.
• Experience in medical settings, behavioral medicine, health psychology, and interdisciplinary teams.
• Knowledge of evidence-based techniques like CBT and Motivational Interviewing.
• Experience with opioid dependence and medication-assisted treatment preferred.

Education

• Licensed Psychologist or Licensed Clinical Social Worker licensed in Pennsylvania.

Salary

• Commensurate with experience and education.
• Grade 21.
