Job summary

Cahaba Medical Care Foundation is seeking a Behavioral Health Consultant for our Wilcox County Wilcox County clinic in Camden, AL. The role focuses on providing behavioral health consultation across age groups to improve psychosocial functioning within an integrated primary care setting.

The position requires a Master's in Social Work or Counseling and Alabama licensure or supervision under a licensed clinician.

Qualifications

Master's Degree in Social Work or Counseling required.
Licensed in Alabama as LMSW/LICSW/ALC/LPC or under supervision of LICSW/LPC/PhD; unlicensed individuals must be supervised.
Excellent working knowledge of behavioral medicine and evidence-based treatments.

Responsibilities

Provide comprehensive assessment and diagnosis of behavioral health clients.
Develop effective treatment plans and assist clients in achieving goals.
Evaluate crisis situations and apply appropriate interventions.
Participate in meetings supporting integrated healthcare model.
Assist in detecting at-risk patients and developing prevention plans.
Help primary care team with care management, guidelines, disease management, and patient education.
Provide assessment, consultation, and brief intervention for psychological/psychiatric problems.
Educate patients, families, and staff on care, prevention and treatment improvements.
Monitor the behavioral health program and recommend improvements.
