Cahaba Medical Care Foundation is seeking a Behavioral Health Consultant to provide integrated behavioral health services at West End Medical in Birmingham, AL. The role emphasizes assessment, treatment planning, crisis intervention, and collaboration with the primary care team to enhance patient outcomes.

Ideal candidates hold a Master's in Social Work or Counseling and be licensed in Alabama (LMSW/LICSW/ALC/LPC) or eligible for supervision.

Qualificações

• Master's degree in social work or counseling clearly stated.
• Licensed in Alabama (LMSW/LICSW/ALC/LPC) or eligible under supervision.
• Excellent knowledge of behavioral medicine and evidence‑based treatments.
• Ability to work in a fast-paced primary care setting with an interdisciplinary team.
• Knowledge of psycho-pharmacology and care pathways.

Responsabilidades

• Provide comprehensive assessment and diagnosis of behavioral health clients.
• Provide effective treatment planning and assist clients in achieving goals.
• Evaluate crisis situations and apply appropriate interventions.
• Participate in meetings to support Cahaba’s integrated healthcare model.
• Assist in the detection of “at risk” patients and develop prevention plans.
• Assist the primary care team with care management, guidelines, disease management, case management, and patient education.
• Provide assessment, consultation, and brief intervention for psychological/psychiatric problems.
• Teach patients, families, staff care, prevention, and treatment techniques.
• Monitor the site’s behavioral health program and propose improvements.
• Other duties as assigned.
