Qualifications

• Master's degree in social work or counseling clearly stated.
• Licensed in Alabama (LMSW/LICSW/ALC/LPC) or eligible under supervision.
• Excellent knowledge of behavioral medicine and evidence‑based treatments.
• Ability to work in a fast-paced primary care setting with an interdisciplinary team.
• Knowledge of psycho-pharmacology and care pathways.

Responsibilities

• Provide comprehensive assessment and diagnosis of behavioral health clients.
• Provide effective treatment planning and assist clients in achieving goals.
• Evaluate crisis situations and apply appropriate interventions.
• Participate in meetings to support Cahaba’s integrated healthcare model.
• Assist in the detection of “at risk” patients and develop prevention plans.
• Assist the primary care team with care management, guidelines, disease management, case management, and patient education.
• Provide assessment, consultation, and brief intervention for psychological/psychiatric problems.
• Teach patients, families, staff care, prevention, and treatment techniques.
• Monitor the site’s behavioral health program and propose improvements.
• Other duties as assigned.
