Job Overview

The Behavioral Health Consultant (BHC) is a vital member of the healthcare team, assisting care managers in managing the overall health of their enrolled population. The BHC aims to improve recognition, treatment, and management of psychosocial, behavioral, and medical conditions. They provide clinical mental health consultation services within their state's licensure scope to all referred patients, participating in managing chronic and acute diseases, applying behavioral principles, and facilitating mental health and substance abuse assessments, interventions, and referrals.

Responsibilities
  1. Member of Care Team & Population Health Efforts
  2. Develop clinical pathways and best practice programs for targeted patient groups
  3. Participate in staff meetings and trainings
  4. Provide education and support on behavioral health and substance abuse topics
  5. Participate in daily huddles to identify behavioral health needs
  6. Offer population-based care to high-risk patients
  7. Deliver services to all enrollees, regardless of mental health diagnosis
  8. Engage in system-wide learning collaboratives
  9. Promote ongoing change efforts around mental health and substance abuse
  10. Document in the medical record accessible to the entire care team
  11. Engage patients via telephone and in person
  12. Provide immediate assessment, intervention, and referrals during clinic visits
  13. Offer therapeutic counseling using 15-30 minute interventions
  14. Identify functional outcomes and make targeted recommendations
  15. Use solution-focused, CBT, Motivational Interviewing, and other short-term interventions
  16. Coordinate with patients' families and support systems (with permission)
  17. Provide follow-up and relapse prevention education
  18. Promote skill-building activities, sometimes in group settings
  19. Assist patients in crisis with urgent mental health and substance abuse needs
  20. Refer patients to specialized mental health and substance abuse services as appropriate
  21. Advocate for patients perceived as difficult or non-compliant
  22. Re-engage non-participating patients, primarily via phone
  23. Encourage active patient participation in care and connect them to community resources
Qualifications

Minimum requirements include:

  • Master’s degree in a Mental Health Field
  • At least 2 years of experience with evidence-based counseling, screening, diagnosis, and treatment of mental health and substance abuse disorders
  • Knowledge of the relationship between medical and psychological health
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC); out-of-state license transfer within 60 days of hire
  • CPR certification from the American Heart Association
  • Act 34, Act 33, and Act 73 FBI Fingerprinting Criminal Background Clearances
