Overview

A BHC is a member of the core healthcare team who assists care managers in managing the overall health of their enrolled population. The BHC aims to improve recognition, treatment, and management of psychosocial/behavioral problems and medical conditions. They provide clinical mental health consultation within their state's licensure scope to all referred patients, participate in managing psychosocial aspects of diseases, and serve as the team’s mental health and substance abuse expert.

Responsibilities
  1. Participate as a core care team member to develop clinical pathways and best practices.
  2. Engage in practice meetings and training sessions.
  3. Provide education on behavioral health topics to the multidisciplinary team.
  4. Participate in daily huddles to identify behavioral health needs.
  5. Deliver population-based care to high-risk patients based on registry data.
  6. Offer services to all enrollees, not only those with diagnosable mental disorders.
  7. Contribute to system-wide learning collaboratives.
  8. Promote change around mental health and substance abuse issues within the system.
  9. Document care in accessible medical records.
  10. Engage patients via phone and in person, providing assessment, intervention, and referrals as needed.
  11. Provide therapeutic counseling using 15-30 minute interventions, focusing on functional outcomes.
  12. Coordinate with families and support systems with patient permission.
  13. Conduct brief follow-ups, relapse prevention, and skill-building activities.
  14. Assist in urgent mental health crises and refer to specialty services when necessary.
  15. Advocate for patients and reach out to re-engage non-engaged patients.
  16. Connect patients to community resources addressing social determinants of health.
Qualifications

Minimum requirements include a Master’s degree in a mental health field, 2 years of experience with evidence-based counseling, knowledge of medical-psychological health relationships, and licensure as an LCSW or LPC. Additional certifications include CPR and background clearances.
