Job Details

The Integrated Behavioral Health Consultant (LCSW/Clinical Psychologist) is embedded within primary and specialty care teams to work directly with the team to assess individuals with behavioral health and/or substance use issues and to provide brief treatment services within the primary/specialty care setting. The BHC is involved in assessment, planning, implementation, facilitation, monitoring, and evaluation of the options and services required to meet an individual's health needs. They serve as patient advocates and counselors, perform psychosocial assessments as per SBIRT Program Objectives, and refer patients and families to Behavioral Health & Primary/Specialty care resources within the Einstein Healthcare Network. Through effective communication and resource identification, the BHC promotes quality outcomes, patient satisfaction, cost-effective care delivery, and safe transition of care. The ideal candidate is knowledgeable about gender-nonconforming identities and expressions, and the assessment and treatment of gender dysphoria, including evaluating patients for Behavioral Intervention & Referral to Treatment as appropriate.

Job Description

ESSENTIAL RESPONSIBILITIES:

  1. Interview patients and gather information relevant to developing a care plan for disposition. Document patient screening questionnaires from SAMHSA and enter documentation into the e-portal.
  2. Contact behavioral health and social service agencies to gather information, after obtaining patient/legal guardian consent, and connect the patient with these resources, either within or with external partners.
  3. Provide a summary of findings to the attending provider and document the same in the patient's medical record.
  4. Collaborate with the attending provider and clinical team to develop a care plan for disposition and document the consultation in the medical record.
  5. Facilitate patient discharge or transfer to the next most appropriate level of care.

Qualifications

Education required: Master's degree in social work, Counseling, PhD/PsyD in Clinical Psychology, or related social science field.

License required: LCSW, LPC, PsyD/PhD in Clinical Psychology, or equivalent.

Experience required: Two to three years in an integrated healthcare, primary care, behavioral health program, or facility.

Preferred experience: Care coordination, utilization/case management, discharge planning, mental health, or chemical dependence certification.
