Grande Ronde Hospital in La Grande, OR is seeking a Behavioral Health Consultant to join a collaborative care team with primary care providers. The role focuses on facilitating behavioral changes to improve physical, emotional and mental health.

The position requires a Master’s in a relevant field, Oregon QMHP eligibility, and current associate licensure. You will perform assessments, provide therapy, document in the EHR, and coordinate care with internal and external providers.

Qualifications

  • Master’s degree in social work, counseling, or MFT; QMHP qualifications per Oregon rules.
  • Current Oregon associate-level licensure (CSWA, Professional Counselor Associate, or MFT Associate).
  • Must practice under clinical supervision as required.
  • Experience in physician practice preferred.
  • Minimum 2 years mental health clinical experience.
  • AHA Basic Life Support (BLS) certification.

Responsibilities

  • Perform comprehensive behavioral health assessments (psychosocial history and mental status).
  • Develop DSM-5-TR-based diagnostic impressions and treatment plans.
  • Provide individual, family, and group therapy as appropriate.
  • Document services in the EHR in compliance with regulations.
  • Collaborate with primary care and integrated care team for whole-person care.
  • Coordinate referrals to community resources.
  • Engage in regular clinical supervision and apply feedback.
  • Monitor progress and adjust treatment plans as needed.
  • Support PCP behavioral health integration standards.
  • Maintain knowledge of state/federal payer regulations.

Description
Position Summary:

The Behavioral Health Consultant functions as a core member of the collaborative care team that involves the patient’s primary care provider and other community providers of mental health care, addiction treatment, and alternative health care. The Behavioral Health Consultant is responsible for assisting the primary care provider in facilitating behavioral changes in patients that improve physical, emotional and mental health.

Qualifications:
  • Master’s degree in social work, counseling, marriage and family therapy; must meet Oregon Administrative Rule qualifications for QMHP.
  • Current Oregon associate-level licensure required – one of the following:
    • Clinical Social Work Associate (CSWA)
    • Professional Counselor Associate
    • Marriage and Family Therapist Associate
  • Must be able to practice under appropriate clinical supervision as required.
  • Previous experience with physician practices preferred.
  • Prefer minimum 2 years’ of mental health clinical experience.
  • Current American Heart Association Basic Life Support (BLS) certification.
Primary Duties and Responsibilities:
  • Conduct comprehensive behavioral health assessments, including psychosocial history and mental status examination.
  • Develop diagnostic impressions and treatment plans within scope of training, consistent with DSM-5-TR criteria.
  • Provide individual, family, and/or group therapy services for mental health disorders.
  • Document all clinical services in a timely and compliant manner in accordance with state, federal, and organizational requirements.
  • Collaborate with primary care providers and integrated care team members to support whole-person care.
  • Coordinate care with internal and external providers, including referrals to community resources.
  • Participate in regular clinical supervision and apply feedback to support professional development and quality of care.
  • Monitor patient progress, update treatment plans as needed, and adjust interventions based on clinical outcomes.
  • Support compliance with Patient-Centered Primary Care Home (PCPCH) behavioral health integration standards.
  • Maintain knowledge of and adhere to applicable state, federal, and payer regulations related to mental health and substance use treatment.
Skills and Abilities:
  • Strong clinical assessment and counseling skills.
  • Knowledge of behavioral health diagnoses and evidence-based treatment approaches.
  • Ability to establish rapport and maintain therapeutic relationships.
  • Excellent communication and interpersonal skills.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Ability to develop knowledge of related state and federal regulations regarding mental health.
  • Proficiency with electronic health records and computer systems.

Compensation:

Salary $88,500.00, Commencement Bonus of $10,000
