Qualifications

Job description

Essential Duties:

  1. Provide onsite consultation, education, brief assessment, and evidence-based interventions to address chronic disease and acute behavioral health needs in a primary care medical setting.
  2. Provide Behavioral Health Consultations including individual, couples, and/or family sessions, such as:
  • Functional and strength-based assessment and diagnosis;
  • Psychoeducation for patients and support systems;
  • Medication adherence counseling and disease self-management;
  • Motivational Interviewing for behavioral strategies;
  • Problem-solving cognitive interventions;
  • Self-Care Plan development and skills training;
  • Substance use/abuse evaluation and harm reduction strategies.
  1. Work a flexible schedule, including unscheduled, same-day, and scheduled patient visits.
  2. Build therapeutic relationships, perform assessments, and develop negotiated care plans with patients and providers.
  3. Conduct group classes and visits to promote skills and health education.
  4. Provide consultation to clinical staff based on the Primary Care Behavioral Health model.
  5. Assist in treatment planning, implementation, and monitoring of biopsychosocial plans.
  6. Determine need for specialty mental health services or termination.
  7. Respond immediately to psychiatric emergencies with appropriate interventions.
  8. Use DSM, ICD, and other diagnostic systems in assessments and diagnosis.
  9. Provide psychotherapy as needed.
  10. Assist with care coordination and resource access.
  11. Make referrals and follow up on service delivery.
  12. Maintain productivity and manage case flow, assessments, and crisis interventions.
  13. Develop and maintain effective relationships with patients and families.
  14. Promote communication with community agencies and participate in meetings and QA activities.
  15. Implement policies, develop records, and perform other duties as assigned.

Qualifications:

Education/Experience: Master's degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or related field; minimum two years of counseling experience, including cognitive behavioral interventions. Experience in primary care settings preferred.

Language Ability: Bilingual in English and Spanish required. Able to interpret medical and professional documents and communicate effectively with groups.

Reasoning Ability: Capable of problem-solving, data collection, and independent work.

Certificates and Licenses: Valid Texas license (LCSW, LPC, LMFT, or Psychologist) or eligibility through reciprocity.

Computer Skills: Proficiency with internet, Outlook, PowerPoint, Word, Excel, and electronic health records.

Other: Valid driver’s license, access to a car, and ability to travel as required. Ability to perform physical tasks including lifting up to 25 pounds.

Work Environment and Physical Demands: Office and residential settings, with physical activities such as bending, sitting, standing, and walking. May require evening or weekend work.
