Basic Function

Responsible for providing same‑day behavioral health diagnostic assessment and interventions for patients of FCHC.

Job Title: Behavioral Health Consultant ~ LCSW

Primary Responsibilities
  1. Collaborates with the Primary Care Providers (PCPs), BH Care Managers, and other members of the team (RNs, Mas, CHWs) in the delivery of comprehensive and integrated behavioral health care services.
  2. Provides same‑day behavioral health consultation for patients at FCHC, as requested by their PCP, including behavioral assessment, diagnostic evaluation, and brief intervention.
    • Clinical Practice Skills
      • Define problem accurately and use best practices to implement intervention rapidly.
      • Triage high‑risk concerns and refer to appropriate level of care.
      • Conduct assessments with a solid understanding of DSM‑5‑TR, family systems, the connection between physical and mental health, and the impact of trauma on patients and families.
    • Consultation Skills
      • Tailor PCP recommendations to the primary care setting; focus on recommendations that support the provider’s treatment of BH in primary care.
      • Conduct effective curbside consultations.
      • Employ knowledge of BH medications to help PCPs treat complex BH conditions and substance use disorders, including consultation with psychiatry (internally, MOCPAP).
      • Write clear, concise chart notes that meet billing guidelines and facilitate effective team care.
  3. Meets the target number of encounters per year agreed upon with the supervisor and attaches the appropriate billing code to each encounter.
  4. Provides presentations to PCPs on BH topics of interest.
  5. Networks with graduate programs in social work to develop our BH workforce.
Periodic Duties
  1. Contributes services to community public health activities as appropriate.
  2. Participates in Center work team as needed.
  3. Participates in Center staff problem solving groups.
  4. Performs other duties as assigned.
Working Relationships

Inside Center: All Inclusive.

Outside Center: Patients, community organizations and agencies, professional colleagues and organizations, and institutions of learning.

Qualifications
  • Master's Degree in Social Work required.
  • Active MO state license (LCSW) required.
  • Minimum of two (2) years experience working in the field of social work or behavioral health care required.
  • Work experience or strong desire to work in urban community health setting preferred.
  • Strong interpersonal and communication skills required.
Continuing Education
  • Active professional development as demonstrated through attendance of seminars, continuing education programs, etc. is strongly recommended.
  • Satisfactory maintenance of continuing education standards as established by the Division of Professional Registration of Missouri is required.
