Job summary

Union Community Care is seeking a licensed behavioral health clinician to implement counseling, brief interventions, and solutions-focused services for individuals and families within a coordinated care model.

You will work as part of a Team-Based care unit to maximize the level of care, perform intake histories for addiction treatment, and collaborate with clinicians providing treatment. Strong clinical license and English proficiency are required.

  • Current clinical license to practice in Pennsylvania (SW/MFT/PC or Psychology)
  • Excellent communication (verbal and written) skills
  • Ability to work with diverse people and within a team
  • Ability to multitask, prioritize, and meet deadlines

Responsibilities

  • Provide behavioral health counseling as part of a multidisciplinary team
  • Conduct intake histories for addiction treatment and assist with visit planning
  • Collaborate with clinicians to treat addiction and coordinate referrals
  • Document evaluations, treatment plans, and discharge procedures per organizational standards
  • Deliver individual and group counseling to patients across age groups
  • Participate in team and staff meetings and maintain clinical competencies

Each of us is a neighbor, a friend, a family member, and together, we are a trusted community health center.JOB SUMMARYUnder the direction of the Director of Behavioral Health, implement behavioral health treatment, brief interventions andsolutions-focused direct diagnostic and therapeutic services to individuals and families. Provide behavioral healthcounseling as a member of a multidisciplinary group of healthcare delivery professionals providing care in a coordinatedfashion and empowered to work at the top of their professional training.SPECIFIC JOB DUTIES1. Works with individuals, families, and groups to address and treat emotional and mental illnesses and to promotewellness.2. Works closely with members of Team-Based care unit to maximize the level of care.3. Performs intake histories on patients seeking addiction treatment and assists with clinician visit planning.4. Collaborates with clinicians providing treatment for addiction. Assists with patient follow-up and referral toexternal partners for addiction treatment when appropriate.5. Provides diagnostic and therapeutic services in accordance with general standards applicable to mental healthprofessionals.6. Provides individual and/or group counseling services to children, adolescents, adults, elderly, and families.7. Adheres to and upholds the ethical standards and guidelines of the profession.8. Documents work in accordance with Union Community Care standards in the areas of diagnostic evaluations,progress notes, treatment plans, and discharge procedures.9. Participates in education and other activities necessary to maintain competency in clinical work.10. Actively participates in clinical team and staff meetings.11. Performs other work-related duties as assigned.POSITION REQUIREMENTSA current clinical license to practice as issued by the Pennsylvania State Board of Social Workers, Marriageand Family Therapists and Professional Counselors or, a current license to practice Psychology by the Pennsylvania State Board ofPsychology.Excellent communication (verbal and written) skills.Ability to deal effectively with a variety of people and work in a team environment.Ability to multi-task, prioritize and work under deadlines.English proficiency requiredESSENTIAL FUNCTIONSIn order to fulfill the requirements of this position, duties 1-11 considered essential functions of the job.ORGANIZATIONAL INVOLVEMENTThis position is required to participate in mandatory all staff meetings, team meetings and trainings.
