Qualificações

• High school diploma required.
• 1–4 years experience in a social services or nonprofit environment.
• Flexibility to work evenings and weekends.

Responsabilidades

• Provides ongoing assessment of consumers’ wants and develops service plans and reviews every 180 days.
• Assist consumers in crisis situations and accessing benefits.
• Help consumers locate housing and community resources; accompany to important appointments.
• Support consumer transportation and manage symptom self-management and medication adherence.
• Maintain confidentiality and adhere to ethical standards; document services and write progress notes daily.
