Qualifications

• Shallbeanindividualwhoiscertifiedbythe DepartmentasaMentalHealthRehabilitationTechnician/Community (MHRT/C & MHRT-C CSP preferred).
• CADC or LADC preferred.
• Two years of relevant experience with adults in behavioral health case management.
• Working knowledge of local community resources.

Responsabilités

• Serve as the initial point of contact for individuals seeking services.
• Enter client data into the electronic health record system.
• Conduct screenings and assessments; facilitate consent-to-treat forms.
• Coordinate referrals, scheduling, and transitions of care.
• Develop relationships with community providers and outreach partners.
• Ensure documentation and eligibility verification are completed.
