ThedaCare in Neenah, Wisconsin is seeking a Behavioral Health Collaboration Manager to coordinate mental and physical health care for an assigned caseload alongside the primary care team. You will liaise with a psychiatric consultant and broader care team to optimize patient outcomes.

Qualificações

• RN licensure and/or licensure in behavioral health fields.
• Experience with screening and treatment planning for mental health disorders.

Responsabilidades

• Screen and assess patients for mental health and substance use disorders.
• Coordinate care with primary care and psychiatric teams.
• Track treatment outcomes and document encounters in the registry.
• Facilitate referrals to services inside or outside the organization.

Bachelor’s Degree in Nursing (BSN) or Master’s Degree in Social Work or related behavioral health field
