Job summary

CommUnityCare is seeking a Behavioral Health Consultant to integrate behavioral health care within a fast‑paced medical clinic. You will collaborate with primary care providers, specialists, and residents to assess and treat mental health concerns, while maintaining strict confidentiality and high-quality patient care.

The role requires a Master’s degree in a related field, active Texas licensure or eligibility, and minimum two years of clinical mental health experience.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or related field.
• At least 2 years of clinical mental health experience.
• Current Licensure as LCSW or LPC in the State of Texas.

Responsibilities

• Collaborate with multidisciplinary care teams to provide integrated behavioral health care.
• Document treatment plans and progress in the electronic health record.
• Deliver psycho-education and brief interventions to patients and families.
• Provide care coordination across primary care and specialty services.
