Job summary

Hartford HealthCare is seeking a Licensed Behavioral Health Clinician at Windham Hospital. You will work with interdisciplinary teams to deliver evidence-based clinical services across the continuum of care, upholding ethical standards and professional codes.

The role requires a Master’s degree in a related field, CT licensure, and at least two years in a psychiatric setting. Must possess strong communication skills and cultural competence to address health disparities.

Qualifications

• Master’s degree in social work, counseling, marriage and family therapy or related field.
• Two years’ experience in a psychiatric healthcare setting.
• Current Connecticut LCSW, LPC, LMFT, or LADC license.
• Valid Driver's License.
• Strong verbal and written communication skills.
• Cultural competence and awareness of health disparities.

Responsibilities

• Conducts comprehensive bio-psychosocial assessments and creates a care plan.
• Provides direct patient care including case management, care coordination, and crisis planning.
• Refers patients to appropriate levels of care and resources.
• Maintains timely patient-centered documentation per regulatory requirements.
• Collaborates with team to improve patient experience.
• Provides social work field supervision to second year students.

Job Summary

The Licensed Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. The Licensed Behavioral Health Clinician also maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.

Key Responsibilities

• Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
• Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning.
• Refers patient to appropriate levels of care and available resources that promote the individualized care plan.
• Maintains timely patient-centered documentation that reflect the patient and client systems’ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.
• Works as a team member to contribute to improved patient experience.
• Provide social work field supervision to second year social work students

Qualifications

• Master’s degree in social work, Counseling, Marriage and Family Therapy or related Human Service field.
• Previous experience in a healthcare setting.
• Current Connecticut LCSW, LPC, LMFT, or LADC.
• A minimum of two years’ experience in a psychiatric setting required, preferably in an emergency department/crisis role
• Highly effective verbal and written communication skills
• Is cognizant of health-related social needs and cultural competence. Recognizes and addresses health disparities-inequalities and injustices directed toward clients, related to access to care and provision of health services.
• Valid Driver's License.
